Housing is Healthcare model helps people stay housed and healthy
Flagstaff Shelter Services (FSS) is a low-barrier, rural shelter on a mission to end homelessness in Coconino County, Arizona. Serving people 18 and older regardless of race, sobriety, mental illness, and criminal background, this emergency shelter is committed to helping clients transition into permanent housing.
Through Mobilize AZ grant funding, FSS launched the Housing is Healthcare model focused on improving participants’ health and stability, while decreasing emergency room use. Housing-focused Care Teams provide a bridge to physical health services, substance use treatment, and recovery services for those without stable housing (a key social determinant of health).
In just one grant year of wraparound support with both health and housing, clients’ lives dramatically improved. Of clients served, 81% reduced substance use, 60% improved previously untreated behavioral and physical health needs, and 43 people were permanently placed in housing. Housing is Healthcare’s robust model has quickly become a best practice for Flagstaff Shelter Services. In collaboration with organizations providing emotional and behavioral support, FSS will continue to meet the needs of the people they serve in a holistic, integrated way in support of long-term health and stability.
