Arizona’s first health navigation and transportation system for the unhoused and underserved
Elaine is an organization that conducts street outreach and connects Arizona's unhoused and underserved (e.g., low income, uninsured, older adults) to social and community services vital to their health and well-being. By providing accessible transportation combined with advocacy in navigating and receiving needed services, it reduces barriers to healthcare, food, employment, and housing.
Funding from Mobilize AZ has increased Elaine’s capacity to serve the most vulnerable members of our community — especially those needing care for behavioral health or substance misuse. During the grant period, Elaine provided 373 people with 1,379 rides to appointments and necessary resources, including getting to the grocery store, picking up life-saving medications, getting to and from work and housing interviews, and so much more. Grant funds also enabled Elaine to build strategic alliances with other organizations serving at-risk populations, such as Community 43, a multidisciplinary behavioral health outpatient clinic in central Phoenix.

Elaine’s services resulted in a 95% appointment adherence rate for clients. With consistent access to necessary health services, their health outcomes and food security significantly improved. Nine individuals were able to obtain and sustain employment, and 10 people are no longer experiencing homelessness.
Elaine’s community impact reveals that properly designed transportation and health navigation systems can improve safety, link the underserved to opportunity, connect the isolated and disabled to crucial services and social support, and stimulate economic development. Elaine is committed to growing its vehicle fleet and strategic collaborations to help more people access the critical services they need — with the ultimate goal of reducing homelessness and health disparities in Arizona.
